HealthCare Partners Signs Agreement with Care1st to Provide Care for Cal MediConnect Patients in Los Angeles County

Torrance, Calif., October 10, 2014 -- /EPR NETWORK/ -- HealthCare Partners announced today it signed a provider agreement with Care1st Health Plan to care for patients in Cal MediConnect, a new program for patients eligible for both Medicare and Medi-Cal.

Care1st is one of five health plans in Los Angeles County participating in Cal MediConnect, the three-year pilot program that integrates all medical and pharmaceutical care, behavioral health care and long-term services and supports under a coordinated care plan.

HealthCare Partners will manage all medical care for patients in Cal MediConnect while also working with Care1st to actively participate in the management of long-term services and supports. Both organizations bring extensive resources to the partnership to fully coordinate the behavioral health needs of this population.

The agreement is pending Department of Managed Health Care (DMHC) approval.
